Waiver:
In consideration for the opportunity to receive permanent jewelry services from Bonds & Bling, LLC, participants hereby acknowledges and agrees to the following terms:
1. Assumption of Risk: The participant understands that the process of receiving permanent jewelry involves the use of a flameless arc welder and other tools. The participant acknowledges that there are inherent risks associated with this activity, including but not limited to minor burns, skin irritation, or other unforeseen reactions.
2. Release and Waiver: The participant, on behalf of themselves and their heirs, executors, administrators, and assigns, releases and discharges Bonds & Bling, LLC, its owners (Maya Trujillo, Pamela Trujillo, Johnny Trujillo, Johnny R. Trujillo), employees, agents, and representatives from any and all claims, liabilities, demands, actions, causes of action, costs, and expenses, whether at law or in equity, whether known or unknown, arising out of or in connection with the participant's receipt of permanent jewelry services.
3. Voluntary Participation: The participant acknowledges that their participation in the permanent jewelry service is voluntary, and they have the opportunity to ask questions and seek clarification about the process before proceeding.
4. Weld Guarantee: The participant understands that Bonds & Bling, LLC provides a weld guarantee, and in the event that issues arise with the permanent jewelry, the participant agrees to contact Bonds & Bling, LLC promptly to arrange for necessary repairs.
5. Photographic Release: The participant grants Bonds & Bling, LLC the right to use photographs taken during the permanent jewelry service for promotional and marketing purposes.
6. Compliance with Instructions: The participant agrees to comply with all instructions provided by Bonds & Bling, LLC regarding the care and maintenance of the permanent jewelry, including any aftercare instructions provided.
